![]() We are committed to providing our patient with the best care possible. If you have medical insurance we will assist you in receiving your maximum allowable benefit. However, your insurance is a contact between you and your insurance company. You need to be aware of your carrier's rules, regulations, and payment policies as some services may not be covered. You are responsible for the timely payment of your account in the event your insurance does not make within 60 days. Patient Information: All new patients must complete our Patient Information Form before seeing the doctor. It is the responsibility of the patient to keep GVG informed of updated information such as insurance carriers, new primary care Physicians, personal addresses, and phone numbers. PAYMENT EXPECTATIONS: The following applies to charges that are the responsibility of the patient. (Does not apply to charges covered by the insurance carrier.) Co-payments are collected by the front desk at the time of service. If the patient does not pay, the physician will be notified, and it will be determined if the patients needs to be rescheduled. No show fees of $20.00 will be assessed if the appointment is not canceled 24 hours prior to the scheduled visit. Exceptions will be made for extenuating or emergency situations. A $25.00 charge will be assessed on checks returned from the bank as non-sufficient funds. After the third check the patient will be on a cash, money order, or credit card payment basis. Balances:
Less than $250.00 - Total paid within 90 days (minimum of $50.00) All payment arrangements will be signed by the patient. A copy will be given to the patient and one will be kept on file at GVG. In the event that the payment agreement is broken, the balance will be due immediately, which could result in the account being turned over to collections. Exceptions can be based upon individual need with the authorization of the billing supervisor or practice manager. CollectionsOur billing staff is always willing to work with our patients according to individual needs and financial situations. It is with great reluctance that an account is sent to collections for payment. However, once this decision is made by the billing supervisor and the physician, serviced in out office can not be rendered until the account is paid in full or consistent payments are being made the collection bureau. (Exceptions are emergency care or ongoing medical treatment on the orders of the physician).
