New Patient Information

Plan to arrive at lease 15 Minutes early to park and register with the receptionist. There is a hospital parking facility available for your use.


Please be sure the referring Physician has sent the appropriate information to our office including any referrals if you are required to have one. Your medical records are very helpful to the Doctor and, as such are essential on your first visit.


If any lab work is needed specimens are sent to the lab(s) that we feel do the best work. Because of this our patients may be responsible for a higher cost of service. If you or your insurance prefers your labs to go to some place other than Spectrum and Pathology Lab, please let us know.


Payment is required when you come in for you appointment, unless our office participates with your insurance. If you have co-pay insurance, please be prepared to pay at time of your visit. If you need to make special arrangements for payment, call the billing department before you scheduled appointment.


Make sure to bring any paper work sent and your most recent insurance card. If no insurance card is present you will be responsible for payment at time of service.


Our office does our best to keep your scheduled appointments, however our Doctors could be called out of the office for Deliveries or Emergency Surgery at any time. We do our best to keep you informed of this and to accommodate you in any way we are able.
